This is a dual 40mm fan duct for the Ultimaker 2 E3Dv6 mount designed by lions3. It will only work with this mount.
E3Dv6 Ultimaker 2 mount:

To mount it, you will need to drill an tap 2 4-40 or 3mm holes on each side of the hotend mount.
