I made this piece from a bunch of other designs to make it fit my AWorldnet a200 printer with a E3D V6 head and proximity switch.
The holder is from Prusa i3 Bowden extruder with inductive 18 mm proximity sensor auto level REMIX! by lazzorblade. and the base from Prusa i3 A602 Metal X carriage
by 118d. feel free to mod and use.
