2:1 extruder gear set for Mendel

UPDATE 04 Dec. 2015:
- Reduced module for more stepper motor adjustement
- Fixed nut housing height

These will change the ratio of the extruder from 5:1 to 2:1 for improved feed speed.
As the original parts were loose, these have been designed with no gap, so it may need some sanding and run-in.
On my Mendel, I had to set E to 388.5 steps/mm (original was 875 steps/mm).
Pinion locking screw is M3. Pinion should fit snuggly a NEMA 14 stepper.
