Proximity Sensor Mount (12mm diam) for E3D v6 integrated fan support

This 12mm diameter sensor mount is designed to be used with shuttle3d's "E3D v6 integrated fan layer":

The combination of these two parts allows a PLA cooling fan and an inductive proximity sensor to be mounted directly to an E3D v6 (or similar) heat break.
