Lumia 640 tripod mount

I needed a holder to use my phone to document testing. This holds the phone horizontally with mass balanced over the tripod. Slide a 1/4-20 nut into the slot and mount on a tripod. Worked well when printed on a Makerbot Replicator 2 in PLA.
