Extruder Motor Fan Mount / Filament Guide - Printrbot Simple

During long print sessions I was having issues with my extruder motor overheating, resulting in the filament melting at the drive gear. I checked around thingiverse and, while there were many types of fan mounts, I didn't see one that worked for me. I needed the fan to be elevated so it was oriented properly to blow across the heatsink that I had attached to the extruder motor. (see pictures).
