This is a Z Axis end stop screw thumb adjuster for use with the Improved Y-Axis Gantry End Blocks by 'thedayowl' (http://www.thingiverse.com/thing:995185) when using the Print-Rite DIY 3d printer.
I used a heated bed spring to keep it tight and in the bottom of the Y Axis part I used a captive nut, this solves all the issue with the thread being damaged when adjusting the Z Axis end stop (I think the photo of the captive nut is shown upside down).
