E3Dv6 Boden mount with proximity sensor (18+12mm)

A bowden mount for the E3Dv6/light6 (and similar designs) with support for a proximity sensor. Screw spacing is 30mm and fits this carriage http://www.thingiverse.com/thing:889407
as well as all other standard 30mm carriages.
All the parts can be printed without support EXCEPT for the base of the 18mm one.

Offsets for the 18mm are X=36 and =-2.5
