Precision Adjustable Z Endstop Holder and Constraint

A precision adjustable optical endstop holder for Prusa printers, with an integrated Z screw constraint bearing.

I got frustrated with trying to adjust my Z axis endstop by sliding the clamp up and down the smooth rail, and found that the available designs for adjustable clamps wouldn't work for me, for one reason or another.
