Best printed laying on its side for maximum strength.

Fasten GoPro with a long zip tie to go around the camera and the mount as shown on the pictures. The lip on the top bracket will secure the camera from ejecting.
Use the side support brackets to keep the camera from sliding side to side and keep the camera centered in the mount.
