Print the base upside down and the holder sideways.
Mount the upper part to the base either with 2 M4 screws (10mm) or melt in M3 nuts in the upper part (these special melt-in nuts, I love them...).
Use the two chassis screws at the corner to fix the mount.
Depending on the cam you use you need a regular cam fixture screw, or print this:

(This screw is actually too small in diameter for me and the thread does not fit).
