Somewhat based off of this http://www.thingiverse.com/thing:2026848 but completely reworked to use a larger screw and nut to better keep its position. Used the thumb screw from here http://www.thingiverse.com/thing:1443439.
1x M5 nut
1x M5 50mm bolt (with hexagon head)
The widening cap goes on the bottom of the bolt so that it doesn't have to be perfectly fit to actuate the switch.
If your printer is like mine, there is VERY little space at the end where this would like to mount. I had
