This is a totally functional, and almost-totally-printed click pen that I designed for an engineering interview. It's almost-totally-printed because you need to find a spring and ink cartridge elsewhere.

As you can imagine the tolerances on the parts need to be exact for the pen to function. Sharpen the edges of the teeth of the gear and plunger so that they are as fine as possible. Also lightly sand the different surfaces of the gear until it slides smoothly into the screwtop and plunger.
