Motor mount for the plates:

There is no a thread (M6) for the teflon tube fitting. It could be tightly turned into the plastic 5mm hole.
It was made to use together with Frankenstein extruder:

UPDATE:
There was not enough of original spring power. You have to add a spacer between the level arm and spring.
