Printrbot Simple Metal Cable Chain E3dD Titan Mount

With the E3D Titan & pancake nema 17, the extruder mount for the cable chain won't fit. As a remedy, this integrates the cable mount portion onto the E3D Titan mount/

Update: I got tired of the titan leaning forward as it would let the hot end have a lot of play. I added a shim to the file to fix this.
