E3D Titan + V6 hotend mount for Mendel and Prusa 50mm rail distance

I couldn't find a mount for the E3D Titan extruder + their V6 hotend for my MendelMax 1.5 printer, so I designed my own. It's designed to be used with Igus drylin RJ4JP-01-08 bushings.
