This is the Pokemon Sun/Moon Island Challenge amulet; I modelled it for a friend's cosplay as an accessory! I'll update this with print photos/info once I print one, but it should be fairly trivial to print with whatever settings you like.

There's two holes on the sides of the bottom for the stringy bits to be attached, and one big hole through the middle for a string to be looped through and wrapped around.
