E3D Low Profile mount with 40mm fan

This is a combination of ideas - the low profile design of http://www.thingiverse.com/thing:154938 and the 40mm fan mount of http://www.thingiverse.com/thing:94086.

I bought an E3D Bowden hot end for my Prusa Mendel & loved it, but it cut my usable height to ~70mm. Also, I didn't have 24v available for the 30mm fan, so I wanted to use a standard, quiet 40mm with decent airflow. Now my girlfriend can sleep & I can print up to ~145mm

I purposefully left the mounting holes off the model
