This is a V6 heatsink cooling fan shroud for mounting 5015 centrifugal/radial fans horizontally. Use a screw to secure the fan to the shroud and the shroud itself snaps onto the heatsink.
Print as-is, or with supports. Bridging worked fine for me, but ymmv.
