Smartrap jhead attach with 40mm fan mount

This a modded version of the jhead attach part that has a 40mm fan mount. Test it out and let me know. I have printed one out on my Prusa i3 but have not tested it out yet. I tried to keep it simple as possible so the fan is mounted a little higher than it needs to be.
