Customizable Name Tag

This is remix of "Simple Customize Name Tag" by rsiripong. I've simply changed the OpenSCAD code so that rather than there being only hard-coded values, I've made them in to variables at the start of the file so you can somewhat easily completely change the look of the whole thing.
