LM8UU bushing with 20mm spacing holes for Prusa i3 carriage

Slic3r was giving errors when slicing [this thing](http://www.thingiverse.com/thing:94256), therefore I remade it in Openscad using Prusa's original scad file.

I moved the ziptie hole a bit higher and allowed space for captive m3 nuts.
