Here you will find a model to carry your inductive proximity sensor either 8mm or 12mm. Even though I have added a model for 16mm sensor I have no way to test it as mine is only 12mm in diameter.

Mounting holes are designed to be attached on a 30mm fan as I have one on my Tevo Tarantula.
