This part is made to cool both the heat sink and apply a curtain of air around the nozzle for cooling the part. It is was originally made for the E3D V6 but I have used it with a few different extruders now. When you tighten it down it seems to bow up a little bit which pushes the back end of it closer to the print bed but I haven't seen any performance issues with this.
