Direct drive Extruder for E3D/J-Head with BLTouch mount

I remixed ffleurey's great extruder design by adding a mount for an Antclabs BLTouch Sensor at the left side, giving enough room to add this great cooler: https://www.thingiverse.com/thing:1973156 ;
I also modified the idler slightly, by adjusting some dimensions and adding a "cap" for the ball bearing axis, so that it can't bend anymore under the spring tension, also holding the ball bearing right in place. It can be fastened with a self-tapping screw.
