Dual fuse holder for RAMPS 1.4

I was inspired by the original simple and slick design by neo777. So I decided to modify it a bit: combined two holders into one piece. It simplified printing in vertical orientation and PCB mounting. Standard ATO fuses and 6.3 mm spade connectors were used. Spade connectors were trimmed with Dremel in order to make pins.
