This is an adapter mount/shroud for 40x40x10mm Noctua Hotend fan. This shroud will fit an E3D v6 Hotend heat sink.
UPDATE: 4/6/19
Updated the model slightly to increase grip and added guide extrusions so that the model doesn't slide up and down on the heat sink.

Extended the base by 1mm to increase structural strength at the bottom
