E3D volcano 40mm fan mount and layer fan funnel

i saw this but it did not fit right..so i redid it
1. the fit around the heat sink is tight to get a good seal for airflow of the heatsink

2. added some thickness to it and gussets/tapers/fillets around high stress areas and corners

3. made a notch at the bottom right of the cylindrical area and tapered the notch at the bottom to allow heater cables to come out and not push the heater cables down into the corner of the heater block, reducing risk of breaking the wires or cutting through
