This is a E3D Titan Aero mount for the Ender/CR-10 x carriage, with a layer fan (4020 blower) and BL Touch mount.

I made the PCB as a breakout board for my own use but decided to share in case anyone wanted to use it, you should attach removing the screws in your nema17 and using longer screws with a spacers between the back of the motor and the PCB.
