One-off Extruder: Afinia/UP! motor -> JHead

Finding myself with a spare working Afinia extruder motor (and the block that goes on it). I wanted to combine it with a Ubis hotend which uses a jhead mount.

mounting holes are present for 6-32 SAE screws (used with https://github.com/Xssink/Prusa_i3x/blob/master/STLs/X_carriage_(Print_1).stl) and 50mm spacing.
