Printed in Printrbot simple 2014. 0.4mm nozzle 0.2mm layer PLA.
I suggest using over 50% infill and 3 exterior layers for strength.
You will also need 1 M4 screw without head, 3 short M5 with nuts, 1 M6 bolt and 4 long M5 with nuts.
Works great.
