Parametric flexible heterogenous shaft coupler

This is an OpenSCAD parametric flexible shaft coupler, based on the one by thinkyhead, but with the ability to set different shaft sizes for each side of the coupler. It's also cleaned up a bit to allow for more easily re-using the coupler module.
