Rostock Max Low Profile Fan Remix

Recently added an E3D V6 hotend to my Rostock Max. Tried a couple of different layer fan replacements, but had some problems with them interfering either with the arms or with the heads of the modified bolts I'm using. Started wondering if I could remix one of the designs into something that would work for me, and this is the result.

dreyfusduke's design relies on having the effector platform upside down (apparently for LED mounting), but mine is right side up, so the mounting tab is ~5.5mm
