Lulzbot dual extruder fan mount

The Lulzbot dual extruder tool head does not come with cooling fans! This is a single fan mount for the rear extruder (I'm using the FlexyDually so I only need cooling on the non-flexible filament extruder). It bolts into the brass inserts on the right side of the tool head, and uses the same 40mm fan as the stock single extruder tool head.
