Another Cr-10 Hotend Fix

Another cr-10 hot end fix washer.

Added a little more finesse to Chuck's washer, so the two tubes are aligned when assembled. There are some 4.2mm guides to align the tube better. Also a slight taper added to the inside to centre it on the throat.

Needs to be printer at very fine layer height to get the most out of the features.
