Get some old Backup Tape Sleeve from your IT-guys, check the type (important for the size - it should be an LTO Ultrium 2 Tape Sleeve)
Choose the needed version (they differ in the amount of slots), print and glue them in.
Be aware that the sleeve is out of PP which means that you'll need some special glue for it.
