This is a remix of an existing base (https://www.thingiverse.com/thing:520101 thanks to stingraydoctor) scaled up to fit with the trophy (Premier League Trophy https://www.thingiverse.com/thing:1380509 by Badgerous) along with a plaque to be attached to the base with the wording of your choice.
.FCStd file should be editable in FreeCAD to modify the wording.
