This is a gear cable adaptor which allows the use of modern, handlebar mounted shifters on classic frames that only accept classic, frame mounted shifters.
As you can see by how dirty the frame is, it has lasted a long time with no issues.
This is for frames with a down tube diameter of 29mm and a classic shifter mount (The square bit attached to the frame) width of 12.5mm/(1/2").
