Captive Customisable Cable Holder Flat Surface Mounting

This is a remix of [bitswype's](https://www.thingiverse.com/bitswype/about) [Captive Customizeable Cable Holder](https://www.thingiverse.com/thing:145187/), which in turn is a remix of [xfile's](https://www.thingiverse.com/xifle) [Customizable Cable Holder](https://www.thingiverse.com/thing:130495).

I wanted to mount the cable holder with double sided tape under my table. Setting the tab length to zero removed them, but then the thumbwheels would touch the desk. I created some distance by extending the rectangular block on the top.

You can set the distance between desk and thumbwheel with desk_distance = X; on line 15 in addition to all the other settings from the original.
