By popular demand I am posting the puppet design I have used for DVD menus for the show Mystery Science Theater 3000. It is designed for easy, supportless printing. Enjoy!

Assembly is pretty straight forward. CA (superglue) the parts together. Use filament to register where you see registration holes. You will need a plastic, wood, or brass rod for the main shaft. 1/8th inch for the 100% version and slightly smaller for the 70% size. There is a hole for a string and a slot to put a small rubber band in the mouth.
