Designed using OpenSCAD for toothpaste tube sizes up to around 8oz and 65mm width.
Printing: I printed using PETG at 0.20 layer height, 25% infill, and brim enabled. No supports needed. SCAD file is included if you want to make any customizations to the dimensions, shape, handle style, etc. Components are in one file, but can easily be split apart in slicer software and printed separately for higher quality or different infill/etc.
Assembly: Press fit the twister rod into the main body completely (past the retainer tabs).
Usage: Toothpaste tube should be inserted into the twister rod from the side after at least 10% of the tube is consumed, and the knob should then be twisted in your favorite (consistent) direction. To remove the empty tube, turn it slightly in the reverse direction to loosen and then pull it out from the side; no need to unroll it completely. (Printing with higher resolution and fewer strings may make it easier to remove)
Update 4/6/20: Added roun
