A 3-channel Dew custom heater controller for telescopes. I used 3 "WayinTop" PWM voltage regulator that were 32mmx32mm. They snap in nicely.
DC jacks (in and out) were standard panel mount 5.5x2.1mm jacks. Had to ream the holes a bit to get them to fit. Internal wiring is up to you - I'd recommend some diodes on the input side of the PWM to keep things clean as the PWM controllers make a bit of noise on the inputs, but it's really just a case of wiring them in parallel.
4xM4 3/8" cap screws to hold it together.
Dew Heater straps are nichrome wire sandwiched in electrical and duct tape, inside a sewn sleeve. foam insulation on top and folded aluminum foil on bottom for even heating. Guidescope strap is shown - about 48 ohms worth for ~3W max.
Any sufficiently-sized 12VDC input (I use a 2A, that's enough for my needs).
