DM: you come to a dead end, at the base of the wall there is what appears to be a few bones and something shiny laying on the floor.

Player: I examine and search the bones.

DM: As you bend over to examine the bones, there is a wavering in the air and on the wall in front of you. A triangular mouth filled with sharp glistening teeth and three jagged mandibles appears from the ether at exactly your current head level. Roll a perception check to see if you notice it.
