This is an everyday tool used to avoid direct contact with door handles and physical buttons. Over the revisions I've narrowed on a few key features.
The round feature at the back of the tool is for the index finger during use and, when not in use, hanging from a belt loop for easy access.
The extended feature on the bottom of the round is to protect the middle finger from slipping forward and allow more control of the tool when pushing.
The middle triangular feature is to strengthen the part when pulling or pushing, a straight feature is too thin and likely to snap. The opening in the middle also adds more continuous perimeter layers which also increases the rigidity of the part.
The front hook feature is slanted to aid in pulling handles. Also, at the very front is a small flat, roughly the size of a fingertip, to be used for push buttons.
