Reworked CTC i3 top mount

This is a poorly merged, but functional piece for the CTC Prusa i3. It is a trainwreck between https://www.thingiverse.com/thing:1694197 and http://www.thingiverse.com/thing:1369178.

The latter item doesn't fit the CTC rework, but provides the bolt holes for adding a spool mount to the top of the printer - https://www.thingiverse.com/thing:1427114

I have not printed the stand yet so I don't know if it needs to be longer, owing to the extra height in how the LCD is mounted to the printer. Might relocate the LCD.
