Remix of IAMGROOT8888's bow tie modified to fit on the outside a Bivona pediatric tracheostomy tube.
The hole in the center is 17 mm - that's big enough to go over the weird bumpy part and sit flush with the neck.
Scale down slightly for it to fit a Shiley pediatric tube, those don't have the weird bumpy part.
I don't know how this would work on an adult trach tube, I don't have access to one.
Use a speaking valve / cap / HME to keep the tie from falling off.
