sn04 adapter for he3d prusa i3

This is a very simple adapter for the sn04 sensor.
The provided design has parts that slide into eachother, but the precision of the printer is simply not good enough for this. After 2 hours of filing and grinding, I figured there had to be a beter way. This is that better way.
