Hyper Integrated Z axis

I frankened this together from many ideas to make a part that kills quite a few proverbial birds.

It's derived mostly from triffid_hunter's http://www.thingiverse.com/thing:12151. Adds a lift to the Z motor like, ROTORIT's http://www.thingiverse.com/thing:12620. Then I added a 608 bearing to keep the z leadscrew in line and arms to support a wade style extruder.
