Machining is probably best for this.
Measurements taken from the adaptor that comes with Shutter Precision/Exposure QR15 dynamo hubs.
It's a bit of a loose fit to the nominal 15 mm ID on the hub. You can certainly make it to better tolerances. I just copied what I measured.
You could also just turn a cylinder of OD 15 mm, ID 12 mm, & length 100 mm to step to a 12 mm skewer.
