E3D Titan to V-Slot Mount - 7mm spacing

This is (another) mount for attaching the Titan + stepper motor to a V-Slot rail. Unlike other mounts found here, this one has 7mm spacing between the stepper and the extruder.
E3D provide screws with the Titan for 2 spacing choices - 2mm and 7mm. I found that using 2mm meant that I was unable to attach the extruder gear grub screw to the flat on my stepper axle (the flat isn't milled all the way down the axle).
